How to add new provider from the admin panel?

Description:
It will display the add new provider form.
If you can able to add the new providers in this page.
In add provider, it should be required fields for
Name - Add the provider name.
Email - Add the valid email address.
Mobile - It should be 6 to 13 digits.
Password - It should be min 6 digit values.
- Admin Panel → Providers → Add Provider
