You can add the provider subscription plan details on this page.
Some fields are mandatory to add a provider subscription.
Title - Title of subscription for example:Free, premium.
Plan - Add the how many month is valid your plan.For example(1 to 12 Month)
Amount - Add here your provider subscription amount value.
Description - It is optional part for description brief you plan details here.
Admin Panel -> Provider Subscriptions -> Add Subscription
